We have been using the GetFeedback surveys for a long time and like them. Salesforce now requires everyone to use enhanced domains. The GetFeedback development app has a link that is supposed to go to the Salesforce outbound message and flows. It does not work for Salesforce sandbox applications that use enhanced domains.

Great Survey Tool
We moved our survey from our ITSM tool into Salesforce after integrating all of our service data into the Force.com platform,. Now surveys are triggered in, distributed by, and results are compiled and displayed all in Salesforce. The surveys are slick and engaging, and we are tracking NPS and CSAT among other things on every customer, case, and technician.
Very nice product to generate good lookioing, Salesforce integrated, surverys
As a nonprofit - $$ cost is always an issue - the basic subscription we have, for now, seems to fit our needs pretty well. To get the same functionality from SurveyMonkey would cost a bit more $ and probably give us more than we need. Creating a survey is really pretty easy. After you learn how to set up your survey links, you an capture the responses you want and feed them back into Salesforce - passing information from the source object to related objects one example is a survey on a training class - we have the class/event - that class has an instructor and all the students and maybe a venue - the id for the instructor can be passed through the survey to get a 'rating' on that person and then on a related list put that back on the contact record for that person, or venue etc. So now not only do I have a survey for the class - but I have feedback history I can see right on that instructors contact record.
