The CareJourney app gives Health Cloud users a new way to harness provider performance data for key business objectives, such as: ● Curating high performing networks – search database based on cost/quality quintiles, location, or practice group affiliation and add providers in Health Cloud at the click of a button ● Referral management – care coordinators can match dozens of provider-specific sub-specialty classifications and performance information on over 1000 different condition and treatment episodes with a patient’s needs to make smarter referrals ● Contracting with provider groups and payers – use descriptive statistics like panel size, cost/quality quintiles, practicing areas, and performance in specific episodes relative to market benchmarks to make contracting or outreach decisions Available Metrics: - Provider Sub Specialty - Condition/Treatment Episodes - Provider Cost/Quality Scores - Panel Size - Risk Score - Benchmark Where is the data derived from? The data is derived from a combination of CareJourney’s access to full Medicare and commercial claims data to provide insights across 270+ million lives and about 2 million providers, nationally.
